Health forms are due August 13.
The completed forms MUST be approved by the nursing office or our athletic trainer prior to tryouts. Athletes will NOT be allowed to participate in tryouts or practices without CURRENT forms. Current means the physical exam has been completed within the immediate past 12 months.
